Introducing our care navigators
We now have members of the practice team who are trained as care navigators. They can:
- Ensure that you are helped quickly
- Signpost you to the most appropriate person or service who can help you
- Help you over the phone or face to face in the practice.
They may signpost you to a person or service within the practice as well as other NHS organisations, social care and in the community.
You may find that you get asked a little more information than normal – this is so we can help make sure that you get the help and support that you need, first time.

HEALTH EDUCATION SERIES:
Lunchtime Lectures – Come along to the practice to listen to speakers covering a range of topics. February looked at Men’s Health with over 40 people attending. This talk was delivered by Dr Nigel Twelves and well received. Our next topic is on Vascular Health – 11th March 2020. You don’t have to be a patient at Ponteland Medical Group.
Ponteland Medical Group and our Patient Participation Group invite you to attend our patient education series on Vascular Health. Dr Matthew Thomas and Dr Rachel Taylor will be discussing preventing cardiovascular disease and peripheral arterial disease.
